Job Title: Veteran Support Specialist

Job Summary:

Simple Steps LLC is seeking a highly motivated and compassionate Veteran Support Specialist to join our team. As a key member of our staff, you will play a vital role in providing high-quality home and community services to our Veterans with neurological disorders, physical disabilities, and traumatic brain injuries.

Key Responsibilities:

  • Deliver high-quality home and community services to our Veterans, ensuring their needs are met and exceeded.
  • Develop and implement personalized care plans, working closely with Case Managers and family members to ensure effective support.
  • Provide education and training to clients and their families on health care plan services and procedures.
  • Ensure accurate and complete documentation, including tracking SMART goals and progress.
  • Collaborate with healthcare professionals, clients, families, and the general public through verbal and written communication.
  • Transport clients during visits, using reliable transportation and maintaining agency-required auto liability insurance.

Requirements:

  • High School Diploma or equivalent.
  • Experience working with physical and cognitive impairments/disabilities, traumatic brain injuries, and post-traumatic stress disorder.
  • Valid driver's license and reliable transportation.
  • CPR/AED/First Aid Certification (American Red Cross or American Heart Association only).
  • Social Security Card.
  • Auto Insurance (100k/300k Bodily Injury).
