Community Support Specialist

Simple Steps LLC is seeking a dedicated and compassionate Community Support Specialist to join our team. As a Community Support Specialist, you will play a vital role in providing high-quality home and community services to our Veterans with neurological disorders, physical disabilities, and traumatic brain injuries.

Key Responsibilities:
  • Deliver personalized care plans to meet the unique needs of each client
  • Support clients in achieving their goals and improving their quality of life
  • Collaborate with Case Managers and family members to ensure seamless care coordination
  • Provide education and support to clients and their families on health care services and procedures
  • Document client progress and maintain accurate records
Requirements:
  • High School Diploma or equivalent
  • Experience working with individuals with physical and cognitive impairments/disabilities
  • Knowledge of Traumatic Brain Injury and Post-Traumatic Stress Disorder
  • Valid driver's license and reliable transportation
  • CPR/AED/First Aid Certification (American Red Cross or American Heart Association)
