Social Services Coordinator

We are seeking a highly skilled and compassionate Social Services Assistant to join our team at Sedgewood Center in Falmouth, ME. As a key member of our social services team, you will provide administrative support to our social service staff, ensuring a seamless transition for patients and residents throughout their stay.
Key Responsibilities- Collaborate with patients and their families to provide support and resource information, addressing their concerns and needs.
- Respond to issues identified by patients and their families, providing timely and effective solutions.
- Facilitate patient transfers within the nursing center, ensuring a smooth transition and adjustment for patients and residents.
- Participate in the healthcare decision-making process, working under the direction of a qualified social worker and/or consultant.
