Role Objective:

  • The Chronic Care Coordinator (CCC) operates within the Chronic Care Management (CCM) program at WC Health. The CCC collaborates with the entire care team to ensure that patients enrolled in the CCM program receive the necessary attention and support to achieve their healthcare goals and effectively manage their chronic conditions. Communication with patients occurs via phone or the CCM platform, where the CCC provides education, care coordination, follow-up, and appointment assistance. The CCC also works closely with Medical Assistants to coordinate care plan requirements and follow-ups.

Key Responsibilities:

  • Develop and implement individualized patient-centered care plans utilizing our CCM software.
  • Maintain regular contact with patients to facilitate care coordination, support, and adherence to the Chronic Care Management program, aiming to enhance positive health outcomes.
  • Ensure ongoing communication with Primary Care or Psychiatric Clinicians regarding urgent patient needs or critical situations.
  • Accurately document all client interactions, both verbal and written, in a timely and efficient manner.
  • Manage a caseload of up to 500 members.
  • Coordinate with scheduling personnel to arrange appointments and verify that all members have completed necessary appointments.
  • Review and process referrals as needed.
  • Monitor members' medication adherence when required.
  • Qualify as many members as possible for billing at the end of the month.
  • Engage with members via phone as necessary.
  • Create and update care plans as required.
  • Review member charts as needed to ensure comprehensive care.

Qualifications:

  • Strong oral, written, and telephone communication skills.
  • Ability to work independently with excellent organizational, collaborative, and time management skills.
  • Familiarity with clinical practices, diagnostic procedures, and treatment protocols.
  • Proficient in applying clinical judgment and critical thinking skills.
  • Bilingual proficiency is an advantage.
  • Capability to build trusting relationships with patients and partner practices.
  • Adept at thriving in a fast-paced environment.
  • Self-motivated with a focus on quality and productivity.
  • Strong problem-solving and consultative abilities.
  • Exceptional customer service skills, as evidenced by positive feedback.

Employment Type: Full-time

Education and Experience Requirements:

  • Preferred qualifications include Medical Assistant, LPN, RN, or 3-5 years of clinical experience.
  • Experience with Electronic Health Records (EHR) software is essential.

Additional Requirements:

  • Successful completion of a drug and background check is mandatory.
