About the Role

We are seeking a compassionate and dedicated Care Manager to join our team at Sunrise Senior Living. As a Care Manager, you will play a vital role in providing high-quality care and services to our residents and their families.

Responsibilities
  • Build Meaningful Relationships: Develop strong relationships with a specified number of seniors and their families, providing assistance with activities of daily living and attending to individual care needs.
  • Notify Management of Changes: Inform management of any changes in a resident's condition and recommend adjustments in the level of care and service as needed.
  • Assist with Life Skills: Assist residents with life skills and other life-enriching activities as indicated on their individual profile.
  • Participate in ISP Development: Participate in the development of Individualized Service Plans (ISPs) and monthly updates.
  • Ensure Resident Safety: Responsible for the safety and well-being of their designated group of residents during the shift, physically checking on them throughout the shift.
  • Partner with Community Team: Collaborate with the community team to ensure compliance with OSHA requirements and promotion of Risk Management programs and policies.
  • Provide Excellent Customer Service: Respond to the dining needs of residents and guests while maintaining Sunrise hospitality and service standards.
Qualifications
  • Dedication to Serving Seniors: Demonstrate a passion for serving seniors with excellent customer service skills.
  • Positive Attitude and Flexibility: Possess a positive attitude and the flexibility to perform various duties in service to the residents.
  • High School Diploma or Equivalent: Hold a high school diploma or equivalent, and may be required to maintain certifications in certain states.
  • Previous Experience Working with Seniors: Previous experience working with seniors is preferred.
  • Ability to Make Decisions: Possess the ability to make choices, decisions, and act in the best interest of the residents.
