We are seeking a compassionate and dedicated Care Manager to join our team at Sunrise Senior Living. As a Care Manager, you will be responsible for providing high-quality care and services to a consistent group of residents and their families in our assisted living and reminiscence neighborhoods.
Key Responsibilities- Build meaningful relationships with residents and their families, providing assistance with activities of daily living and attending to individual care needs.
- Notify management of changes in condition and recommend adjustments in the level of care and service.
- Assist residents in life skills and other life-enriching activities as indicated on their individual profile.
- Participate in the development of Individualized Service Plans (ISPs) and monthly updates.
- Responsible for a designated group of residents during the shift, ensuring their safety and well-being.
- Partner with the community team to ensure compliance with OSHA requirements and promote Risk Management programs and policies.
- Respond to dining needs of residents and guests while maintaining Sunrise hospitality and service standards.
- Dedication to serving seniors with excellent customer service skills.
- Positive attitude and flexibility to perform various duties in service to residents.
- High School diploma or equivalent required; certifications may be necessary in certain states.
- Ability to make choices, decisions, and act in the resident's best interest.
- Competent in organizational and time management skills.
- Demonstrate good judgment, problem-solving, and decision-making skills.
