Care Coordinator Liaison

We are seeking a skilled Care Coordinator Liaison to join our team at Griswold Home Care. As a key member of our team, you will be responsible for presenting our agency as the best home care option, acting as a liaison between clients, referral sources, and our agency.
Responsibilities:
- Presents Griswold Home Care as the best home care option
- Acts as a liaison between clients, referral sources, and our agency
- Schedules and coordinates care for clients and provides support for caregivers
- Recruits and orients staff
- Helps develop client care plans
- Maintains client confidentiality and quality care
- Listsens to clients' and staff concerns
- Makes cold calls, develops leads, and introduces potential leads while promoting the agency
- Is an information resource for clients, caregivers, and referral sources
- Conducts telephone intakes
- Supports the office with scheduling, marketing, interviewing, and administrative duties
- Performs initial home visits with new or potential clients to explain services
- Executes follow-up home visits with existing clients
- Is on call
Qualifications:
- Exhibits strong communication skills and can communicate clearly and concisely in both verbal and written form, fluent in English, articulate
- Has high school or advanced education
- Is comfortable with working and inputting into a computer
- Has a preferred understanding of general care techniques and the home care industry
- Has strong organizational skills
- Has good time management skills
- Is detail-oriented
- Has a valid driver's license, reliable transportation, and auto insurance
- Has a nurturing ability (caring, empathetic, and compassionate)
- Has creative problem-solving skills
- Can work with individuals of varying backgrounds towards a common goal for the betterment of clients
