Care Coordinator/Community Liaison

We are seeking a highly skilled and compassionate Care Coordinator/Community Liaison to join our team at Griswold Home Care. As a key member of our team, you will play a vital role in representing our company and interacting with our clients and caregivers.
Responsibilities:- Presentation and Liaison: Present Griswold Home Care as the premier home care provider, acting as a liaison between clients, referral sources, and our agency.
- Care Coordination: Schedule and coordinate care for our clients, providing support for our caregivers and ensuring the highest level of quality care.
- Staff Recruitment and Orientation: Recruit and orient new staff members, ensuring they are equipped to provide exceptional care to our clients.
- Client Care Planning: Assist in developing client care plans that meet their unique needs and preferences.
- Confidentiality and Quality Care: Maintain client confidentiality and ensure that all care is delivered with the highest level of quality and compassion.
- Client and Staff Support: Listen to clients' concerns and staff concerns, providing support and guidance as needed.
- Community Outreach: Engage in cold calls, develop leads, and promote the agency through effective communication and a positive image in the community.
- Information Resource: Serve as an information resource for clients, caregivers, and referral sources, providing accurate and timely information.
- Telephone Intakes: Conduct telephone intakes to assess client needs and provide initial support.
- Administrative Support: Provide administrative support to the office, including scheduling, marketing, interviewing, and other duties as needed.
- Home Visits: Perform initial home visits with new or potential clients to explain services and execute follow-up home visits with existing clients.
- On-Call: Be available on-call to address client concerns and provide support as needed.
- Communication Skills: Exhibit strong communication skills, able to communicate clearly and concisely in both verbal and written form, fluent in English, and articulate.
- Education: High school or advanced education is encouraged, with on-the-job training provided.
- Computer Skills: Be comfortable working with and inputting data into computer systems.
- Industry Knowledge: Preferred candidate with an understanding of general care techniques and the home care industry.
- Organizational Skills: Demonstrate strong organizational skills, with good time management and attention to detail.
- Valid Driver's License: Possess a valid driver's license, reliable transportation, and auto insurance.
- Nurturing Ability: Display a nurturing ability, with a caring, empathetic, and compassionate approach to client care.
- Creative Problem-Solving: Demonstrate creative problem-solving skills, able to work with individuals of varying backgrounds towards a common goal.
