Position Overview

Role Summary

The Enhanced Case Management (ECM) Program Coordinator II will be instrumental in assisting the Case Manager II by managing the coordination and administration of ECM services. This role is pivotal in ensuring the efficient delivery of case management services, improving client access to essential resources, and fostering communication among clients, community partners, and internal teams. The Program Coordinator will also contribute to the formulation and execution of program policies and procedures, maintaining high standards of service delivery.

Key Responsibilities

The primary duties include, but are not limited to:

  • Program Coordination: Assist in the creation and implementation of ECM program policies and procedures.
  • Collaborate with the Case Manager II to guarantee comprehensive service delivery and resolve any case management-related issues.
  • Enhance communication and collaboration between clients, community partners, and internal teams.
  • Client Advocacy and Support: Aid the Case Manager II in performing client assessments and crafting individualized care plans.
  • Guide clients in accessing and navigating community resources and services.
  • Champion clients' rights and needs within the community, ensuring their concerns are addressed.
  • Administrative Duties: Maintain precise and current documentation of client interactions and service delivery.
  • Track program performance and client progress, providing regular updates to management.
  • Engage in team meetings, training sessions, and professional development opportunities.
  • Community Engagement: Establish and nurture relationships with community partners and agencies to enhance service coordination.

Qualifications

Minimum Requirements (Knowledge, Skills, and Abilities)

  • Bachelor's degree in social work, psychology, or a related discipline.
  • A minimum of two years of experience in case management or a related field.
  • Experience in program coordination or supervisory roles.
  • Familiarity with enhanced case management models and practices.
  • Strong leadership and team management capabilities.
  • Exceptional communication and interpersonal skills.
  • Proficient in case management software and computer applications.
  • Cultural competence and sensitivity to the needs of diverse populations.
  • Ability to work independently as well as collaboratively within a team environment.
