Job Title: RN Case Manager

Location: Long Beach, CA (Hybrid Role)

Schedule: Monday to Friday, 8:00 AM - 5:00 PM

No weekends, no on-calls

Position Overview:
We are looking for a dedicated and skilled RN Case Manager to become part of our dynamic team. This hybrid position is primarily remote, with occasional field visits, particularly within the Long Beach and surrounding areas.

As an RN Case Manager, you will play a crucial role in managing care for Medicare beneficiaries, particularly those aged 65 and older.

Your contributions will significantly affect the lives of underserved communities, aiding in the prevention of complications arising from avoidable health issues.


Key Responsibilities:

Care Coordination:
Connect with patients both virtually and in person to oversee and coordinate their healthcare. Facilitate transitions of care, including follow-up consultations post-discharge with collaborating providers.

Census Oversight:
Monitor daily patient censuses, manage inpatient, emergency room, and skilled nursing facility admissions. Identify at-risk patients and engage them in appropriate care programs.

Collaboration & Outreach:
Conduct outreach to hospital case managers and work collaboratively with various stakeholders, including physicians, social workers, and other team members. Establish and maintain strong professional relationships to enhance patient engagement.

Patient Engagement:
Connect with higher-risk patients, especially those enrolled in the ACO program, to integrate them into high-risk home care initiatives. Telephonic outreach may be necessary.

Qualifications:

Experience: Minimum of 5 years of relevant clinical experience, with at least 3 years in care management within a health plan, home health, or hospice environment.

Background: Strong expertise in case management is essential.

Education: Bachelor of Science in Nursing (BSN) is required. A valid California RN license is mandatory.

Adaptability: Ability to thrive in a fast-paced, evolving environment. This organization is in its early stages, and processes may frequently change, requiring flexibility.

Mission-Driven: A sincere commitment to serving underserved populations, with a focus on preventing health-related complications and fatalities.

Self-Starter: Capable of working independently with minimal supervision while being an effective team collaborator.

Work Environment:
Hybrid position, with a greater emphasis on virtual work than fieldwork. Company-provided laptop and mileage reimbursement available.

Team Structure:
Small team consisting of a physician, social worker, and per diem nurse practitioner. The initial case load will be manageable but may expand as the program grows.
