Position Overview

Role Summary

The Enhanced Case Management (ECM) Program Coordinator II is integral to the successful administration and coordination of ECM services. This role focuses on optimizing the delivery of case management services, improving client access to essential resources, and fostering effective communication among clients, community stakeholders, and internal teams. The Program Coordinator will also be involved in the formulation and execution of program policies and procedures, ensuring the highest standards of service provision.

Key Responsibilities

The primary duties include, but are not limited to:

  • Program Coordination: Collaborate in the creation and implementation of ECM program policies and procedures.
  • Work alongside the Case Manager II to guarantee comprehensive service delivery and resolve any case management-related challenges.
  • Facilitate effective communication and collaboration among clients, community partners, and internal teams.
  • Client Advocacy and Support: Assist the Case Manager II in performing client assessments and formulating personalized care plans.
  • Guide clients in accessing and navigating community resources and services.
  • Advocate for clients' rights and needs within the community, ensuring their concerns are addressed appropriately.
  • Administrative Duties: Maintain precise and current documentation of client interactions and service delivery.
  • Monitor program performance and client progress, providing regular updates to management.
  • Engage in team meetings, training sessions, and professional development opportunities.
  • Community Engagement: Establish and nurture relationships with community partners and agencies to enhance service coordination.

Qualifications

Essential Skills and Experience

  • Bachelor's degree in social work, psychology, or a related discipline.
  • A minimum of two years of experience in case management or a related area.
  • Experience in program coordination or supervisory roles.
  • Familiarity with enhanced case management models and practices.
  • Strong leadership and team management capabilities.
  • Excellent communication and interpersonal skills.
  • Proficient in case management software and computer applications.
  • Cultural competence and sensitivity to the needs of diverse populations.
  • Able to work independently as well as collaboratively within a team environment.
