About the Role

We are seeking a skilled Medical Social Worker to join our Home Health team on a per diem basis. As a Medical Social Worker, you will play a vital role in providing comprehensive care to our patients in the comfort of their own homes.

Key Responsibilities
  • Evaluate and develop personalized plans of care to address patients' emotional and social needs
  • Assess patients' social and emotional states, identifying areas for improvement and developing strategies for resolution
  • Collaborate with interdisciplinary care teams to ensure comprehensive care and effective discharge planning
  • Provide education and support to patients and families, empowering them to manage their health and well-being
  • Supervise and evaluate the performance of Social Work Assistants, ensuring high-quality care and adherence to company standards
Requirements
  • Masters or doctoral degree from a school of social work accredited by the Council on Social Work Education
  • Social Worker licensure in the state of practice (if required by state law or regulation)
  • Valid driver's license, auto insurance, and reliable transportation
  • Proof of current CPR certification
  • Minimum of one year of experience as a social worker in a healthcare setting, home health, and/or hospice
