We are seeking a skilled Medical Social Worker to join our team at CenterWell Home Health. As a Medical Social Worker, you will play a vital role in providing comprehensive care to our patients in the comfort of their own homes.
Key Responsibilities- Evaluate and develop personalized plans of care to address patients' emotional and social needs
- Assess patients' social and emotional states, as well as their medical and nursing needs, in the home setting
- Provide direction and supervision to Social Worker Assistants as needed
- Collaborate with healthcare teams to develop and implement effective care plans
- Assist patients and families in accessing community resources to support their recovery
- Document patient progress and maintain accurate records
- Masters or doctoral degree from a school of social work accredited by the Council on Social Work Education
- Social Worker licensure in the state of practice (if required by state law or regulation)
- Valid driver's license, auto insurance, and reliable transportation
- Proof of current CPR certification
- Minimum of one year of experience as a social worker in a healthcare setting, home health, and/or hospice
