Key Responsibilities:- Provide comprehensive admission, case management, and follow-up skilled nursing visits to ensure seamless care transitions.
- Collaborate with interdisciplinary teams to develop and implement personalized care plans, addressing the unique needs and goals of each patient.
- Confer with physicians to develop and revise care plans, ensuring alignment with patient needs and goals.
- Coordinate care with external providers, including physical therapists, occupational therapists, and home health aides, to ensure a cohesive and effective care experience.
- Monitor patient progress, reporting updates to physicians and clinical managers to inform care decisions.
- Implement patient care plans in conjunction with patients and families, promoting optimal resolution of needs and problems.
- Supervise and oversee the work of home health aides and personal care workers, ensuring accurate completion of documentation and adherence to care plans.
- Discharge patients after consultation with physicians and clinical managers, preparing and completing necessary clinical documentation.
- Diploma, Associate, or Bachelor's Degree in Nursing
- Minimum one year of nursing experience, with strong med-surg, ICU, ER, and acute experience
- Home health experience a plus
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Strong organizational and communication skills
- A valid driver's license, auto insurance, and reliable transportation
