As a Medical Social Worker, you'll play a vital role in our interdisciplinary care team, working closely with patients, families, and healthcare professionals to address the emotional and social needs of our patients.

About the Role

We're seeking a highly skilled and compassionate Medical Social Worker to join our team. In this role, you'll:

  • Evaluate and develop personalized plans of care to address the emotional and social needs of our patients.
  • Assess and address any relationships between a patient's medical and nursing needs, financial resources, and available community resources.
  • Provide education and support to patients and families to help them cope with social and emotional responses to illness or injury.
  • Collaborate with the interdisciplinary care team to develop and implement effective care plans.
  • Supervise and evaluate the performance of Social Work Assistants to ensure high-quality care is delivered to our patients.
Requirements

To be successful in this role, you'll need:

  • A Master's or doctoral degree from a school of social work accredited by the Council on Social Work Education.
  • Social Worker licensure in the state of practice (LICSW required).
  • A valid driver's license, auto insurance, and reliable transportation.
  • Proof of current CPR certification.
  • At least one year of experience as a social worker in a healthcare setting, home health, and/or hospice.
  • Excellent oral and written communication and interpersonal skills.
  • Ability to read, write, and speak fluent English.
  • Knowledge of medications and their correct administration.
  • Ability to organize tasks, develop action plans, set priorities, and function under stressful situations.
