Key Responsibilities- Evaluate and develop personalized plans of care to address patients' emotional and social needs
- Assess patients' social and emotional states, identifying areas for improvement and developing strategies to address them
- Collaborate with healthcare teams to ensure seamless care coordination and effective communication
- Provide education and support to patients and families, empowering them to manage their care and make informed decisions
- Supervise and evaluate the performance of Social Work Assistants, ensuring high-quality care and adherence to company standards
- Masters or doctoral degree from a school of social work accredited by the Council on Social Work Education
- Social Worker licensure in the state of practice (if required by state law or regulation)
- Valid driver's license, auto insurance, and reliable transportation
- Proof of current CPR certification
- Minimum of one year of experience as a social worker in a healthcare setting, home health, and/or hospice
