Job Description

Job Summary:

We are seeking a highly skilled and compassionate Navigator/Social Services Assistant to join our team at Peoples Family of Corporations. As a key member of our department, you will play a vital role in providing exceptional support to our patients, families, and staff.

Key Responsibilities:

  • Perform daily clerical and administrative tasks to maintain an efficient office environment.
  • Develop and maintain office systems to ensure smooth operations and effective communication.
  • Work collaboratively with staff to facilitate departmental goals and objectives.
  • Monitor and track patient status, referrals, and transitions of care.
  • Support Behavioral Health Integration (BHI) Project Coordinator in educating stakeholders on SBIRT process and patient advocacy.
  • Assist in maintaining Locator files and providing in-service training for social service staff.
  • Remain up-to-date with SBIRT guidelines and training.
  • Perform other duties as assigned.

Requirements:

  • High School Diploma/GED or Associate's Degree.
  • Three to five years' experience in secretarial or medical office setting preferred.
  • Good computer skills, including Microsoft Office, and ability to multitask.

Essential Functions:

  • Perform daily clerical and administrative tasks.
  • Develop and maintain office systems.
  • Work collaboratively with staff.
  • Monitor and track patient status.
  • Support BHI Project Coordinator.
  • Perform other duties as assigned.

  • Reports to work on time and meets acceptable standard for attendance.
