As a Referral Coordinator, you will play a critical role in maintaining information flow in the managed care referral process for all contracted managed care health plans.

Key Responsibilities
  • Assist Care Managers with referrals to Post Acute Services to increase efficiency in transition to their next level of care.
  • Perform duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.
  • Develop and maintain effective relationships with staff, doctors, nurses, patients, and managed care representatives.
Requirements
  • High School Diploma, required
  • Bachelor of Social Work, required
  • Work experience in a physician's office or medical institution with exposure to managed care and primary care rules and practices
