Job Title: Navigator/Social Services Assistant

Job Summary:

We are seeking a highly skilled and compassionate Navigator/Social Services Assistant to join our team at People's Family of Corporations. As a key member of our department, you will play a vital role in providing exceptional support to our patients, families, and staff.

Key Responsibilities:

  • Develop and maintain office systems to ensure smooth operations and adequate communication of all related calls, faxes, and tasks.
  • Perform daily clerical and administrative tasks necessary to maintain an efficient office, serving patients, staff, and providers.
  • Develop monthly reports for supervisors and maintain tickler files and tracking forms to document and ensure deadlines are met.
  • Monitor the Social Service Task List on NextGen, keep track of negative pre-screenings, and refer positive screenings to BHC/SW as applicable.
  • Support Behavioral Health Integration (BHI) Project Coordinator to educate family, partner, and first responder stakeholders on active involvement in the SBIRT process.
  • Assist BHI Project Coordinator to educate patients to advocate for themselves and provide in-service training for social service staff to coordinate with partners on referrals and transitions of care.
  • Remain up to date with SBIRT Guidelines and training as appropriate.

Requirements:

  • High School Diploma/GED or Associate's Degree.
  • Three to five years' experience preferred, with secretarial or medical office training preferred.
  • Good computer skills, including Microsoft Office, and ability to multi-task and demonstrate organizational skills.

Essential Functions:

  • Performs the daily clerical and administrative tasks necessary to maintaining an efficient office which serves patients, staff, and providers.
  • Develops and maintains office systems which work as a team member with all other staff in the Department to ensure smooth operations and adequate communication of all related calls, faxes, and tasks.
  • Performs all other duties as assigned.

Nonessential Functions:

  • Attire is professional, neat, clean, and appropriate for the work environment.
  • Wears proper identification while on duty.
  • Maintains confidentiality of all hospital and patient information at all times, as observed by peers and management.
  • Reports to work on time and meets acceptable standard for attendance.
  • Attends a minimum of two staff meetings, reads minutes for all staff meetings, and participates in committees as required.
