Key Responsibilities:
  • Develop and maintain relationships with patients, families, and staff to ensure seamless communication and coordination of social services.
  • Assess patient needs and develop personalized plans to address social determinants of health.
  • Collaborate with internal and external stakeholders to ensure timely and effective delivery of social services.
  • Provide education and support to patients and families on available resources and services.
  • Maintain accurate and up-to-date records of patient interactions and service delivery.
Requirements:
  • Bachelor's degree in Social Work, Psychology, or a related field.
  • Minimum 2 years of experience in social services or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced environment and prioritize multiple tasks.
  • Strong analytical and problem-solving skills.
