Job Summary

We are seeking a highly skilled and compassionate Social Services Specialist to join our team at SSM Health. As a key member of our palliative care team, you will provide psychosocial expertise and support to patients and families navigating complex healthcare issues.

Key Responsibilities
  • Collaborate with interdisciplinary teams to provide high-quality palliative care, leveraging your expertise in social determinants of health and psychosocial assessments.
  • Assess patient needs and develop personalized plans to address emotional, social, and behavioral challenges.
  • Provide education and support to patients and families on advance care planning, including health care proxies, living wills, and other relevant documents.
  • Conduct psychosocial assessments to identify strengths and barriers related to social determinants of health, including emotional well-being.
  • Offer anticipatory grief support to patients and families experiencing social, emotional, behavioral, and psychiatric problems during transitions to end-of-life care.
  • Promote healthy discussions between patients, families, and medical teams to facilitate understanding of medical conditions and treatment options.
  • Document all psychosocial interventions, capturing patients' stories, cultural perspectives, socioeconomic status, and religious beliefs.
  • Deliver education on palliative care social work to healthcare professionals and learners.
  • Coordinate with case management, risk management, legal departments, and ethics committees to advocate for best possible outcomes for patients.
Requirements
  • Master's degree in Social Work
  • At least 3 years of job-related experience
  • State-specific professional licenses and certifications (e.g., LCSW, LMFT, LPC)
