Care Planning Coordinator

**About the Role**

We are seeking a skilled RN to join our team at Cottonwood Healthcare and Rehabilitation Center as a MDS Coordinator. As a key member of our care team, you will be responsible for coordinating the overall care planning process for our residents.

**Responsibilities**

* Assess and evaluate the quality of care being given to long-term care residents
* Perform initial interviews and assessments for new residents
* Develop and implement care plans that meet the unique needs of each resident
* Collaborate with multidisciplinary treatment teams, caregivers, and families to ensure seamless care delivery
* Coordinate with specialists within the community to achieve care plan goals

**Requirements**

* Graduate of an accredited nursing program
* Valid, unrestricted RN license in the state
* Previous MDS experience preferred
* Previous experience in long-term care preferred but not required
