**Job Title:** MDS Coordinator RN
**Job Summary:**
We are seeking a skilled and dedicated Registered Nurse (RN) to join our team as a MDS Coordinator at Cottonwood Healthcare and Rehabilitation Center. As a MDS Coordinator, you will play a vital role in ensuring the highest quality of care for our residents by coordinating the Medicare, Resident Assessment Instrument (MDS), and care planning process.
**Responsibilities:**
* Assess and evaluate the quality of care being given to long-term care residents
* Perform the initial interview and assess new residents
* Develop a care plan that has long-term and short-term goals for improvement
* Conduct meetings with multidisciplinary treatment teams, caretakers, and families
* Reach out to specialists within the community to get help in achieving care plan goals
**Qualifications:**
* Graduate of accredited nursing program
* Must have a valid, unrestricted RN license in the state
* Previous MDS experience preferred
* Previous experience in Long Term preferred, but not required
