Care Planning Coordinator RN

**About the Role**

We are seeking a skilled RN to join our team at Cottonwood Healthcare and Rehabilitation Center as a Care Planning Coordinator. This role is responsible for coordinating the overall care planning process for our residents, ensuring their needs are met and their quality of life is maintained.

**Responsibilities**

* Assess and evaluate the quality of care being given to long-term care residents
* Perform initial interviews and assessments for new residents
* Develop and implement care plans that meet the unique needs of each resident
* Collaborate with multidisciplinary teams, caregivers, and families to ensure effective care delivery
* Identify and address any care gaps or concerns

**Requirements**

* Graduate of an accredited nursing program
* Valid, unrestricted RN license in the state
* Previous MDS experience preferred
* Previous experience in long-term care preferred
