Care Planning Coordinator RN

**About the Role**

We are seeking a skilled RN to join our team at Cottonwood Healthcare and Rehabilitation Center as a Care Planning Coordinator. This role is responsible for coordinating the overall care planning process for our residents, ensuring that their needs are met and their goals are achieved.

**Responsibilities**

* Assess and evaluate the quality of care being given to long-term care residents
* Perform initial interviews and assessments for new residents
* Develop and implement care plans that meet the unique needs of each resident
* Collaborate with multidisciplinary teams, including doctors, nurses, and therapists, to ensure comprehensive care
* Communicate with families and caregivers to keep them informed and involved in the care planning process

**Requirements**

* Graduate of an accredited nursing program
* Valid, unrestricted RN license in the state
* Previous experience in long-term care or MDS coordination preferred
