**About the Role**

We are seeking a skilled RN to join our team at Cottonwood Healthcare and Rehabilitation Center as a Care Planning Coordinator. This role is responsible for coordinating the overall care planning process for our residents, ensuring they receive the highest quality care.

**Responsibilities**

* Assess and evaluate the quality of care being given to long-term care residents
* Perform initial interviews and assessments for new residents
* Develop and implement care plans that meet the unique needs of each resident
* Collaborate with multidisciplinary teams to ensure seamless care delivery
* Communicate effectively with residents, families, and healthcare professionals

**Requirements**

* Graduate of an accredited nursing program
* Valid, unrestricted RN license in the state
* Previous MDS experience preferred
* Strong communication and interpersonal skills
