Key Responsibilities- Case Management: Provide comprehensive case management services to clients, including crisis intervention, discharge planning, group and individual work, mental health and substance use disorder counseling, screening, and assistance with meeting basic needs and symptom management.
- Documentation and Reporting: Maintain accurate and timely documentation, meeting minimum billable requirements for direct client services.
- Facilitation and Coordination: Facilitate placement, including pre-placement visits and transportation as needed, and coordinate with physical health providers for whole-person care.
- Shift Coverage and On-Call: Provide shift coverage, including part of the on-call team, to ensure 24/7 support for clients.
- Field-Based Services: Provide field-based services as required by program and client need.
- Confidentiality and Compliance: Maintain confidentiality of client care and adhere to all relevant laws, regulations, and company policies.
- Team Collaboration: Participate in scheduled program meetings and contribute to a collaborative team environment.
- Education: Master's degree or bachelor's degree with four years of experience in social services.
- Driver's License and Insurance: Valid CA driver's license and current auto insurance per contract requirements.
- Experience and Skills: Experience providing mental health and/or substance use disorder services preferred. Cultural competency understanding required. Bilingual preferred. Knowledge of Microsoft Office and EHR software preferred.
