Key Responsibilities
  • Case Management: Provide comprehensive case management services to clients, including crisis intervention, discharge planning, group and individual work, mental health and substance use disorder counseling, screening, and assistance with meeting basic needs and symptom management.
  • Documentation: Maintain accurate and timely documentation of client services, meeting minimum billable requirements for direct client services.
  • Facilitation: Facilitate placement, including pre-placement visits and transportation as needed.
  • Shift Coverage: Provide shift coverage, including part of the on-call team.
  • Field-Based Services: Provide field-based services as required by program and client need.
  • Confidentiality: Maintain confidentiality of client care.
  • Program Meetings: Participate in scheduled program meetings.
Qualifications
  • Education: Master's degree or bachelor's degree with four years of experience in social services.
  • Driver's License: Valid CA driver's license and current auto insurance per contract requirements.
  • Experience: Experience providing mental health and/or substance use disorder services preferred.
  • Cultural Competency: Cultural competency understanding required.
  • Language Skills: Bilingual preferred.
  • Software Skills: Knowledge of Microsoft Office and EHR software preferred.
