The Elizabeth Hospice, with a legacy of 45 years, is the oldest non-profit hospice in San Diego. We are expanding and seeking skilled individuals to contribute to our dedicated team and support our mission. Our services cover all of San Diego County and parts of South Riverside County, operating from various hospice office locations.
Mission Statement: To enhance the quality of life for individuals nearing the end of their journey and for those who are grieving.
Compensation: $26 - $34 per hour, based on experience and years of CNA/HHA licensure.
Shift Differential: Eligible for additional pay when working at least 2 hours between 7 PM and 7 AM.
Work Schedule: Flexible. Candidates must provide availability for 6 shifts per month, including 2 weekend days.
On-Call Requirement: Minimum of 1 shift per month.
Work Location: San Diego and South Riverside Counties.
Travel: Required for patient visits.
Key Responsibilities
- Deliver personal care to patients in their homes, skilled nursing facilities, and residential care facilities, taking into account age-related needs. Fulfill Activities of Daily Living as assigned by the Registered Nurse.
- Coordinate personal care services and schedule visits with patients and their families.
- Monitor and report changes in the patient's condition to the primary Registered Nurse and/or Team Manager as necessary.
- Participate in discipline-specific and all-staff meetings while maintaining continuing education credits for licensure.
- Recognize and respond to the needs of patients and families within the scope of practice.
- Perform related tasks as outlined in the care plan, which may include crisis care staffing, deliveries, light housekeeping, and other assigned duties.
- Accurately document observations in accordance with charting standards.
- Maintain timely patient documentation using electronic charting at the point of care.
- Execute basic skills as outlined in the competencies checklist, policies, and procedures required by the agency.
- Report any changes in condition, unexplained injuries, or concerns from patients, families, or facilities to the primary Registered Nurse or Team Manager promptly.
- Adhere to E-way standards while performing all job duties.
- Completion of post-high school vocational or specialized training.
- Minimum of 6 months of experience (waived with special consideration of Team Manager).
- Preferred experience of 1-2 years in direct patient care.
- Current California CHHA and CNA Certification required.
- Current HHA certification.
- Valid California driver's license, proof of auto insurance, and reliable transportation.
- Spanish-speaking candidates preferred.
- Current CPR certification.
- Current negative TB skin test.
- Proficient in computer skills (word processing), and familiar with calculators, fax machines, and photocopiers.
- Must be able to drive in various conditions, including but not limited to steep or windy roads, night driving, and rural areas, and be comfortable entering homes with pets.
