About the Role

We are seeking a highly skilled and compassionate Senior Patient Care Manager to join our team at Gentiva Hospice. As a key member of our leadership team, you will be responsible for overseeing the day-to-day operations of our hospice sites, ensuring the delivery of high-quality patient care, and managing a team of healthcare professionals.

Key Responsibilities
  • Supervise and manage the performance of assigned team members, ensuring they meet the highest standards of patient care and service.
  • Develop and implement effective strategies to improve patient outcomes, staff engagement, and operational efficiency.
  • Collaborate with the Executive Director and Administrator to achieve business objectives and drive growth.
  • Manage and oversee clinical and administrative functions, including record-keeping and compliance.
  • Respond to phone referrals and inquiries in a timely, professional, and compassionate manner.
Requirements

To be successful in this role, you will need:

  • A degree in nursing and a current license to practice in the state of employment.
  • At least three years of experience in direct patient care, with a minimum of one year in a management role.
  • Excellent communication and interpersonal skills, with the ability to effectively manage and motivate a team.
  • Knowledge of hospice care principles, regulatory guidelines, and managed care principles.
