About the Role

We are seeking a highly skilled and compassionate Senior Patient Care Manager to join our team at Gentiva Hospice. As a key member of our leadership team, you will be responsible for overseeing the day-to-day operations of our hospice sites, ensuring the provision of high-quality care to patients, and managing the performance of assigned team members.

Key Responsibilities
  • Supervise and manage the performance of team members, ensuring timely completion of visits and effective patient care.
  • Manage the site in the absence of the Executive Director or Administrator, ensuring seamless operations and patient care.
  • Oversee clinical and administrative functions, including recordkeeping and compliance with regulatory guidelines.
  • Respond to phone referrals and inquiries in a timely and professional manner, providing compassionate support to patients and families.
  • Participate in on-call nursing schedules and provide direct skilled bedside nursing care in patients' homes.
Requirements
  • Degree in nursing with a current license to practice in the state of employment.
  • Two or more years of experience in hospice care, with a strong understanding of the hospice concept and terminally ill patients and their families.
  • Three years of experience in direct patient care, with a minimum of one year of management experience.
  • Excellent patient assessment skills, good oral and written communication, and thorough knowledge of managed care principles and regulatory guidelines.
