Senior Case Manager Job Description

Overture Home Care LLC is seeking a highly skilled Senior Case Manager to join our team in Dallas. As a Senior Case Manager, you will play a critical role in ensuring our clients receive the highest quality care and support.

Key Responsibilities:
  • Uphold our Core Values: Patience, Dignity, and Discretion.
  • Responsible for monthly meetings at Facilities with Nursing Staff or Community Staff members.
  • Responsible for ensuring client's supervisory visits and initial introductions to caregivers are conducted and documented weekly.
  • Responsible for communicating with the Recruitment Coordinator(s) about staffing needs.
  • Participates in completing on-site home admission assessments for new clients.
  • Coordinate client visits and communicate with the Recruiting and Staffing Coordinator.
  • Responsible for Discharging clients and providing summary. Weekly email to Intake Coordinator and Business Office Manager.
  • Conduct on-site home admission assessments for new clients.
  • Develop and manage comprehensive Care Plans for all clients.
  • Serve as a liaison between clients and caregivers, actively seeking client feedback.
  • Complete competency assessments and validations for caregivers.
  • Maintain caregiver competency records in compliance with state regulations.
  • Provide in-service/staff development programs to meet agency standards and comply with all federal, state, and local regulations.
  • Maintain accurate records of staff education programs.
  • Participate in onboarding presentations as needed.
  • Foster effective employee-client relationships.
  • Maintain the confidentiality of all client care information to protect clients' rights.
  • Assist with caregiver assignments during staff shortages (appropriate training required).
  • Answer staffing phone calls to adjust schedules due to unforeseen events.
  • Be on-call as needed.
Requirements:
  • Previous experience in scheduling, managing, and/or healthcare preferred.
  • Willingness to participate in on-call nights and weekends.
  • Ability to pass an extensive background check.
  • Professional communication skills.
Salary Description:

$60,000 - $65,000 / Year
