About the Role

As a Pediatric Nurse Specialist, you will provide high-quality clinical home care to our clients, working closely with our interdisciplinary team to develop and implement individualized care plans. Your expertise and compassion will enable our clients to thrive and reach their full potential.

Responsibilities
  • Provide skilled nursing care to medically fragile children in their homes
  • Develop and implement individualized care plans in collaboration with our interdisciplinary team
  • Conduct thorough assessments and evaluations to identify client needs and goals
  • Collaborate with other healthcare professionals to ensure seamless care coordination
  • Document client progress and outcomes in accordance with regulatory requirements
Requirements
  • RN/LPN with a current valid license to practice as issued by the State Board of Nursing
  • Current CPR certification
