About the RoleAs a Pediatric Nurse Specialist, you will be responsible for providing high-quality clinical home care to medically fragile children. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities- Provide direct patient care, including medication administration, wound care, and vital sign monitoring
- Develop and implement individualized care plans in collaboration with the patient's healthcare team
- Conduct thorough assessments of patients' physical and emotional needs
- Collaborate with other healthcare professionals to ensure seamless care transitions
- Document patient progress and outcomes in accordance with company policies and procedures
- RN/LPN with a current valid license to practice as issued by the State Board of Nursing
- Current CPR certification
