Job Summary:

InterCommunity HealthCare is seeking a dedicated and skilled Advanced Practice Registered Nurse (APRN) specializing in Family Medicine to provide high-quality, comprehensive care to our diverse patient population.

The ideal candidate will be committed to delivering patient-centered care, managing complex patients across all age groups, and working collaboratively within a diverse healthcare team.

Key Responsibilities:
  • Obtain comprehensive health histories and conduct thorough physical examinations.
  • Develop and implement individualized treatment plans for patients of all ages.
  • Ensure timely closure and signature of patient charts/encounters.
  • Practice disease prevention and maintain continuity of care.
  • Adhere to established departmental and medical center policies, continuous quality improvement objectives, and safety, environmental, and infection control standards.
Requirements:
  • Master's or Doctorate Degree in Nursing from an accredited University.
  • Completion of a Nurse Practitioner program specific to the expanded specialty of Family Nurse Practitioner.
  • Active licensure with the State of Connecticut as an Advanced Practice Registered Nurse (APRN).
  • Board certification as a Family Nurse Practitioner.
  • Current DEA (Drug Enforcement Administration) registration.
