Face to Face Health and Counseling is a non-profit organization committed to promoting economic and health equity for young individuals. We prioritize a strengths-based and trauma-informed methodology, focusing on the needs of young people affected by systemic racism and other forms of marginalization. Since our inception in 1972, we have served over 3,000 youth each year.
JOB SUMMARY:
The Youth Intake and Assessment Specialist plays a crucial role in conducting intakes at SafeZone, our day shelter dedicated to youth experiencing homelessness. This position involves engaging with new youth, gathering essential information, evaluating their immediate needs, and linking them to available on-site and community resources.
Additionally, this role acts as the primary assessor for the Coordinated Entry System for youth in Ramsey County. The Coordinated Entry System serves as a gateway to specialized housing for youth facing homelessness. This position will assist youth who are residing in shelters, outdoors, in unsuitable living conditions, or escaping domestic violence by completing a Coordinated Entry Assessment. This assessment determines their placement on a prioritization list for housing programs based on their unique circumstances and history.
This position entails a commitment of 37.5 hours per week.
RESPONSIBILITIES:
- Conduct intakes with youth entering SafeZone, ensuring accurate and prompt data entry into our youth database (Apricot);
- Connect new youth to services and resources available at Face to Face, particularly addressing immediate needs;
- Perform Coordinated Entry assessments with youth experiencing homelessness, accommodating both appointments and walk-ins;
- Accurately enter assessments into HMIS (statewide database) in a timely manner;
- Assist with daily operations at SafeZone as required;
- Train new staff on the intake and assessment processes;
- Stay informed about updates to CEYS and HMIS requirements, communicating necessary changes to the team;
- Collaborate with county-wide Coordinated Entry partners to monitor the status of youth on the priority list;
- Build and maintain relationships with youth-serving programs both internally and externally;
- Deliver exceptional customer service during intakes and assessments, employing a harm reduction and trauma-informed approach;
- Provide services guided by evidence-based practices for assisting youth in overcoming homelessness;
- Engage with youth respectfully and meaningfully;
- Manage crises and de-escalate situations calmly and professionally;
- Work collaboratively with colleagues, fostering a team-oriented environment; and,
- Perform additional duties as assigned.
- High School diploma or equivalent required;
- B.A. in social services or related field preferred; extensive experience may substitute for formal education;
- A minimum of 2 years of experience working with homeless or under-resourced youth;
- Ability to work independently while exercising sound judgment;
- Experience with positive youth development and harm reduction strategies;
- Strong interpersonal and communication skills;
- High reliability and integrity;
- Positive attitude and a commitment to meeting youth needs;
- Effective crisis management skills and the ability to handle stressful situations;
- Understanding of homelessness and its effects on youth;
- Proven ability to communicate effectively with individuals from diverse backgrounds;
- Good time management skills and the ability to prioritize tasks; and,
- Attention to detail and proficiency in data entry.
LOCATION:
This role requires the employee to be onsite.
PHYSICAL DEMANDS AND WORKING CONDITIONS:

PAY & BENEFITS:
The compensation for this position is $22.50 per hour, based on qualifications. Face to Face offers a comprehensive benefits package, including health, dental, short-term and long-term disability, life insurance, a 403b retirement plan, as well as generous paid time off and holidays.
