Job Summary

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments, medical history reviews, and provide health education to patients in their homes. You will work collaboratively with physicians, case managers, and other healthcare professionals to ensure seamless care coordination and optimal patient outcomes.

Responsibilities
  • Conduct adult/geriatric assessments, medical history reviews, and provide health education to patients in their homes
  • Collaborate with physicians, case managers, and other healthcare professionals to ensure seamless care coordination and optimal patient outcomes
  • Provide services in a variety of venues, including home visits, skilled facility visits, and telehealth visits
  • Document patient information and assessment results accurately and efficiently
Requirements
  • Master's degree in nursing or a related field
  • Current RN and NP or PA licensure in the state of practice
  • Board certification in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant
  • Strong computer skills and familiarity with EMR software
  • Excellent verbal and written communication skills
