Your Role and Responsibilities

As a Nursing Case Manager, you will ensure the delivery of high-quality, patient-centered care by:

  • Managing rehabilitative care to achieve optimal patient outcomes.
  • Coordinating and directing care tailored to the specific needs of each rehabilitation patient.
  • Collaborating with the Rehabilitation Program Director and Facility Case Management Director.
  • Facilitating patient involvement throughout the rehabilitation process.
  • Documenting findings from assessments and communicating patient needs effectively.

Qualifications

We prefer candidates with:

  • A Bachelor of Science in Nursing (BSN).
  • A minimum of 5 years of nursing experience, including 2 years in critical care or 3 years with 2 years in case management.
  • Certification in case management, nursing, or utilization review.
