Role ResponsibilitiesThe Case Manager (CM) is responsible for ensuring high-quality, patient-centered care by overseeing rehabilitative services to achieve optimal outcomes. The CM will coordinate and direct care tailored to the unique needs of each rehabilitation patient, collaborating closely with the Rehabilitation Program Director and Facility Case Management Director.
Key Responsibilities Include:
- Providing orientation to patients, families, and caregivers about the case manager's role, rehabilitation philosophy, and discharge criteria.
- Coordinating patient and family education, encouraging active participation in discussions regarding care plans and goals.
- Implementing treatment plans that align with the patient's strengths and preferences, ensuring their involvement throughout the rehabilitation process.
- Documenting findings from discharge planning evaluations and psychosocial assessments, communicating any needs or preferences of the patient and family.
- Promoting positive patient interactions that enhance the overall patient experience.
- Current RN licensure in the state; BSN preferred.
- A minimum of 5 years of nursing experience, including 2 years in critical care or 3 years in nursing with 2 years in case management.
- Preferred certification in case management, nursing, or utilization review.
- Experience with InterQual is preferred.
- Basic Cardiac Life Support certification required within 30 days of employment.
- Associate Degree or Registered Nurse Diploma is necessary.
