Role Responsibilities
As a Case Manager, you will ensure that our patients receive optimal rehabilitative care tailored to their individual needs. Your responsibilities will include:
- Providing orientation to patients and their families regarding the rehabilitation process, including the role of the case manager and the criteria for continued care and discharge.
- Coordinating educational efforts for patients and families, encouraging their involvement in care discussions and decision-making.
- Implementing treatment plans that align with the patient's strengths and preferences, ensuring their active participation throughout their rehabilitation journey.
- Documenting assessments and discharge planning evaluations, effectively communicating the needs and preferences of patients and families.
- Promoting positive interactions with patients to enhance their experience and satisfaction with our services.
Qualifications
- Current RN licensure in the state with a preference for a Bachelor of Science in Nursing (BSN).
- A minimum of five years of nursing experience, including two years in critical care or case management.
- Preferred certifications in case management or utilization review.
- Experience with InterQual guidelines is advantageous.
- Basic Cardiac Life Support certification must be obtained within the first month of employment.
- Possession of an Associate Degree or a diploma in nursing.
