Role ResponsibilitiesThe Case Manager (CM) is responsible for delivering high-quality, patient-focused care by overseeing rehabilitative services to achieve optimal outcomes. The CM will coordinate and direct care tailored to the unique needs of each rehabilitation patient, collaborating closely with the Rehab Program Director and Facility Case Management Director.
Key Responsibilities Include:
- Providing orientation to patients, families, and caregivers regarding the case manager's role, rehabilitation philosophy, and criteria for continued care and discharge.
- Facilitating patient, family, and caregiver education, encouraging their involvement in discussions about care plans and goals.
- Implementing individualized treatment plans that align with the patient's strengths, needs, and preferences.
- Documenting findings from discharge planning evaluations and psychosocial assessments, communicating any social or financial needs of the patient.
- Promoting positive patient interactions to enhance the quality of service.
- Current RN licensure in the state; BSN preferred.
- A minimum of 5 years of nursing experience, including 2 years in critical care or 3 years in nursing with 2 years in case management.
- Preferred certification in case management, nursing, or utilization review.
- Experience with InterQual is preferred.
- Basic Cardiac Life Support certification must be obtained within 30 days of employment.
- Associate Degree or Registered Nurse Diploma required.
