If you are passionate about enhancing the well-being of others, consider the role of Case Manager (CM). In this position, you will ensure high-quality, patient-centered care by managing rehabilitative services to achieve optimal outcomes. You will be responsible for providing, coordinating, and directing care tailored to the unique needs of each rehabilitation patient.

The CM will collaborate closely with the Rehab Program Director and Facility Case Management Director regarding departmental functions and coordinate efforts within the Rehabilitation team.

Orientation: You will receive training on the case manager's role, rehabilitation philosophy, and criteria for continued stay and discharge. This will also cover Medicare and insurance benefits, grievance procedures, treatment plan processes, and patient rights and responsibilities.

Your responsibilities will include:

  • Implementing the patient's treatment plan that aligns with their strengths, abilities, needs, and preferences.
  • Facilitating patient involvement throughout the rehabilitation process.
  • Documenting findings from the Discharge Planning Evaluation (DPE) and psychosocial assessments.
  • Communicating the social, financial, or discharge needs and preferences of the patient, family, or caregiver.

Qualifications:

  • BSN Bachelor's degree preferred.
  • A minimum of 5 years of nursing experience, including 2 years in critical care, or 3 years of nursing experience with 2 years in case management preferred.
  • Certification in case management, nursing, or utilization review preferred.
