About the Role
Nuvance Health is seeking a skilled Licensed Clinical Social Worker to join our home healthcare team. As a member of our team, you will work closely with patients, caregivers, and families to provide comprehensive care in the comfort of their own homes.
Responsibilities
- Provide services ordered by the physician as indicated in the plan of care.
- Offer patient, caregiver, and family counseling and education.
- Assist the physician and other team members in understanding significant social and emotional factors related to health problems.
- Participate in case conferences with the healthcare team and refer patients to community resources when necessary.
- Identify patient and family needs for other home health services and refer accordingly.
- Record and report clinical goals and notes to document patient progress.
- Participate in quality assessment and performance improvement programs.
- Fulfill all compliance responsibilities related to the position.
Requirements
- Experience in a qualified home health agency.
- Minimum three years of experience.
Working Conditions
- Little or no manual skills required.
- Some occupational risk.
- Extremely heavy effort may be required.
