Job Title: Social Worker LCSW/LPC

At Danbury Hospital, we are seeking a highly skilled Social Worker LCSW/LPC to join our team in the Inpatient Physical Medicine department.

Job Summary:

The successful candidate will provide behavioral health consultation to adult patients in the acute inpatient rehabilitation unit, working to strengthen patients' coping skills, support successful adaptation and recovery, and facilitate the appropriate and effective management of emotionally related issues.

Key Responsibilities:
  1. Provide behavioral health assessments and treatment, as well as consultation and support to patients' families and significant others.
  2. Support the healthcare team in managing patients' complex psychosocial issues.
  3. Provide individual and family patient-centered support, and psychoeducation.
  4. Establish patient-focused treatment plans to provide disposition and/or referral, identifying potential barriers, limitations, and coping abilities.
  5. Assess patients' clinical readiness for transition in collaboration with the healthcare team and inform the team of available resources based on patients' clinical needs.
  6. Maintain easy flow of communication regarding patients' development and progress to all involved in the care of the patient.
  7. Maintain knowledge of community resources and refer patients and families to agencies for supportive services and concrete resources.
  8. Attend interdisciplinary team meetings to provide behavioral health perspective and input on patient care and treatment planning.
  9. Document accurately, objectively, and timely, patients' plan of care and treatment modalities and interventions.
  10. Collaborate effectively with team members, developing good working relationships both inside and outside of the team.
  11. Fulfill all compliance responsibilities related to the position.
Requirements:

The position requires CT licensure as a Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW). A graduate degree in clinical mental health counseling from an accredited school is also required. Previous experience in a physical rehabilitative setting or related setting is preferred, as is a minimum of 3-5 years of experience.

The ideal candidate will possess strong communication and interpersonal skills, with the ability to work effectively in a team environment. They will also have a strong understanding of behavioral health principles and practices, as well as knowledge of community resources and referral processes.
