About the Role
Nuvance Health is seeking a skilled Licensed Clinical Social Worker to join our home healthcare team. As a key member of our team, you will provide high-quality patient care and support to individuals in the comfort of their own homes.
Responsibilities
- Provide patient-centered care and services as ordered by the physician, in accordance with the plan of care.
- Counsel and educate patients, caregivers, and families on health-related issues and provide emotional support.
- Collaborate with the healthcare team to identify and address social and emotional factors affecting patient health.
- Participate in case conferences and refer patients to community resources as needed.
- Document patient progress and report to the physician.
- Contribute to the agency's quality assessment and performance improvement program.
- Fulfill all compliance responsibilities related to the position.
Requirements
- Experience in a qualified home health agency.
- Minimum of three years of experience.
- Licensed Clinical Social Worker (LCSW) credential.
- Master's Level Degree.
Working Conditions
- Little or no manual skills required.
- Some occupational risk.
- Extremely heavy effort may be required.
