Responsibilities:
- Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
- Develop and implement individualized plans of care to meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure comprehensive care and support for patients.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Document patient progress and outcomes in accordance with agency policies and regulations.
Requirements:
- Current license to practice as a Physical Therapist in the state of employment.
- Minimum of one year of experience as a Physical Therapist in an acute setting.
- Valid driver's license and auto insurance in your name as a driver.
- Ability to meet the physical demands of the job, including lifting, bending, and standing for extended periods.
