BAYADA Home Health Care is seeking a skilled Home Health Physical Therapist to join our team in Washington, District of Columbia.
Job Description:We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our clients in their homes. As a Home Health Physical Therapist, you will work closely with our multidisciplinary team to develop and implement individualized care plans that promote independence, safety, and overall well-being.
Responsibilities:- Conduct thorough assessments and evaluations to identify client needs and goals.
- Develop and implement personalized care plans that address physical, emotional, and social needs.
- Provide one-on-one therapy sessions to clients in their homes, focusing on mobility, strength, balance, and safety.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Document client progress and outcomes accurately and thoroughly.
- Current VA Physical Therapist license.
- Experience in home health care or outpatient settings preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Dynamic and problem-solving skills.
