Home Health Physical Therapist

Alternate Solutions Health Network LLC is seeking a skilled Physical Therapist to join our team in Fairfax, Virginia.
About the Role:- As a Physical Therapist, you will provide comprehensive skilled services to patients in their homes, as ordered by the attending physician.
- You will educate patients and their family members on proper use of equipment and ensure the safety of the patient.
- Accurate documentation and compliance with legal rules and regulations are essential.
- Plan of Care: Develop and implement individualized plans of care for patients, including OASIS assessments and adjustments as needed.
- Patient Outcomes: Treat patients to relieve pain, develop or restore function, and maintain maximum performance using physical means.
- Minimize Patient Risk: Identify and address potential risks to patients, including re-hospitalizations.
- Family Educator/Advocate: Counsel patients and family members on disease management and provide education on preventative measures.
- Supervision: Oversee and supervise therapy services provided by PTAs and nurse assistants.
- Current license as a Physical Therapist in the state of employment.
- Minimum of one year of experience as a PT in an acute setting.
- Valid driver's license and auto insurance.
