Job Summary
We are seeking a highly skilled and compassionate Field Case Manager to join our team. As a Field Case Manager, you will work closely with treating physicians, employers, customers, and injured/disabled individuals to create and implement treatment plans that promote optimal recovery and return to work.
Key Responsibilities
- Develop and implement case management plans that align with the individual's needs and goals.
- Collaborate with treating physicians and other healthcare professionals to ensure comprehensive care.
- Conduct thorough assessments of injured/disabled individuals to identify treatment needs and develop effective plans.
- Communicate with employers and customers to ensure seamless return-to-work processes.
- Monitor and evaluate treatment progress, making adjustments as needed to ensure optimal outcomes.
- Manage workload effectively, prioritizing cases and making decisions to ensure efficient case management.
Requirements
- Active Registered Nurse (RN) license required.
- 2+ years of clinical practice experience, preferably in workers' compensation or related field.
- URAC-recognized certification in case management (CCM, CDMS, CRC, CRRN, or COHN) preferred.
- Valid driver's license and reliable transportation required.
- Ability to work independently and effectively communicate with diverse stakeholders.
