We are seeking a skilled and compassionate LPN to join our team at CenterWell Home Health. As a Home Health LPN, you will provide high-quality nursing care to patients in the comfort of their own homes.
Key Responsibilities- Provide professional nursing services under the direction of a RN in compliance with state and federal regulations.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Confer with RN Supervisor regarding needed changes in the Plan of Treatment.
- Accept verbal orders from physicians and communicate these orders to the RN Supervisor.
- Maintain accurate and detailed records of patient care and progress.
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid drivers license, auto insurance, and reliable transportation.
- At least 1 year of experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
